Overview
Prostate cancer is a condition in which abnormal cells grow in the prostate, a small gland in men located below the bladder that helps make semen. It is one of the most common cancers in men, and in many cases it grows slowly and may not cause symptoms for years. Some prostate cancers, however, can grow more quickly and spread outside the prostate if not detected and treated. Screening tests such as PSA blood testing, prostate MRI, and prostate biopsy can help find prostate cancer earlier and guide treatment decisions. Because prostate cancer can behave very differently from person to person, it is important for men to talk with their doctor about their individual risk, screening options, and the best approach to care.
PSA is a blood test that can help detect changes in the prostate. An elevated PSA does not always mean prostate cancer. PSA can also rise because of an enlarged prostate, inflammation, infection, recent ejaculation, or certain procedures. If your PSA is elevated, the next step is usually to look at your overall risk and decide whether more testing is needed.
There is no single PSA number that means a man definitely has prostate cancer. Your age, prostate size, family history, prior PSA levels, PSA trend over time, and other health factors all matter. A PSA that is rising quickly or is higher than expected for your age may lead your doctor to recommend additional evaluation.
A prostate MRI gives a detailed picture of the prostate and can help identify areas that look suspicious for cancer. MRI can help determine whether a biopsy is needed and can also guide a more targeted biopsy if an abnormal area is found. This can improve the chance of finding clinically significant prostate cancer while helping avoid unnecessary biopsies in some men.
Biomarkers are urine or blood tests that provide more information about a man’s risk of having prostate cancer. These tests can be especially helpful when the PSA is elevated but the decision about biopsy is not clear. Biomarkers do not replace PSA, MRI, or biopsy, but they can help patients and doctors make a more informed decision.
A prostate biopsy may be recommended when PSA, MRI, biomarkers, family history, or other risk factors suggest a higher chance of prostate cancer. During a biopsy, small tissue samples are taken from the prostate and examined under a microscope. This is the only way to know for sure whether prostate cancer is present.
A standard biopsy samples different areas of the prostate in a systematic way. An MRI-targeted biopsy uses MRI findings to focus on specific areas that look suspicious. Many patients benefit from a combined approach, where targeted samples are taken from MRI-visible areas along with systematic samples from the rest of the prostate.
